           DEKALB COUNTRY TRANSITION

           PLANNING COMMITTEE

                                        OVERVIEW

Introduction:

When Public Act 86-1218 Became law in Illinois on August 30, 1990, three crucial areas were identified in the law:

  1.  State Level Planning Activities through the Transition Coordinating Council  (ICC).

  2. Creation of local level Transition Planning Committees (TPCs).

  3. Individual Transition Planning and documentation of annual needs.

Transition  Defined:

A dynamic process involving a partnership of consumers, school-age services, post-school services, and local community members (employers, co-workers, neighbors, friends, and others) that enhance a persons’ Employment, Independent Living, Integration, and Community Participation.

State-wide Mission:

The mission of the state Transition Coordinating Council  (TCC) is to facilitate a statewide process for transition from school to integrated post-secondary education, employment, and community living which ensure that every student with a disability will have the opportunity for community-based services, gainful employment, and community living which will enhance self-determination and independence.  The state Transition Coordinating Council promotes the integration, independence, and productivity of all with disabilities by advocating for, facilitating best practices, and public policy that will lead to successful transition.

DeKalb County Mission:

                                          Formed on October 29,1991, the DeKalb County TransitioPlanningCommittee                      identified the following mission:

  All children, youth and adults regardless of disability, have the right to be educated, work, and play in the community in which they live without bias or restriction. To this end, the purpose of the DCTPC is to assist parents, students, educators, and adult service providers in forming partnerships to plan effectively for individuals in the transition from school to adult live by:

  1. Identifying transition needs 

  2. Assessing transition activities 

  3. Recommending needed changes                 

 

DeKalb County Transition Planning Committee GOALS:

·       A job opportunity will be available for every graduating student who has been receiving special education services in DeKelb County.

·       Accessible transportation and parking

·       The availability of accessible housing

·       The maximum use of natural supports in vocational, residential, and recreational community environments.

 DCTPC Accomplishments:

Development of the "DeKalb County Post-High School Planning Guide."

Creation of the guide for parents entitled "Questions to Ask About: Homes, Jobs, Friends and Fun."
Formulation of a computerized listing of DeKalb County Work Placement Opportunities.
Provided assistance in the expansion of TRANS-VAC transportation service hours for people with disabilities in DeKalb County.
Contributes statistical deta for the state-wide "Transition Outcomes Report."
